Each plan is built around what a child actually needs at that age, so you are not paying for X-rays a toddler doesn’t need yet. Membership runs for twelve months from the day you sign up.

We’ll honor this plan if your child turns 4 before it expires.

  • The annual fee must be paid in full at initial sign-up and again at the time of renewal.
  • The plan cannot be transferred between patients.
  • If you cancel before your renewal date, any visits or procedures already received will be billed at our usual and customary rates, without the
    discount.
